Questions to ponder-

1. Do you have a health care proxy or an Advance Directive. Discuss this with your doctor–it is a covered visit for Medicare patients with no copay.

2. If you were unable to make decisions for yourself who
would make decisions on your behalf?

3. Have you filled out a MOLST form?

4. Your health records are protected after your
death for up to 10 years- is there a personal representative you wish to
disclose it to, after your death?
